30 minute follow up - $150
60 minute follow up - $200

Our care team will attend to you and gather basic information to set up an appointment.
Prior to your visit our insurance department pre-verifies your coverage to ensure a stress free visit. Monarch Psychiatry takes a variety of insurances.

Emotional Support Animal Letter- $80 per pet
Benefits/FMLA (1-3 pages)- $80
Benefits/FMLA (+ 4 pages)- $100
